This position has access to and knowledge of extremely sensitive, private and confidential materials.
Ability to maintain the highest standard is required with zero tolerance.
All the primary duties within this document will be performed according to established policies, procedures and guidelines within the department.
Responsible for coordinating home care services which includes establishing and maintaining the patient's plan of care.
Responsible for the quality, customer service and financial management of assigned case load.
PRIMARY JOB DUTIES: Assesses patient needs through the collection and interpretation of health care assessment data.
Develops and ensures all patient plan of cares are appropriate to meet the individual needs of the patient.
Coordinates patient care between the patient, caregiver, physician, and other appropriate disciplines.
Responsible for the completion of all home health documentation, including the timeliness of admissions, recerts, discharges, and all OASIS documentation.
Ensures all clinical records are in compliance with health care law, reimbursement guidelines, agency policy and accreditation standards.
Obtains, clarifies, documents and reports physician orders as appropriate.
Provides patient and caregiver teaching as appropriate per the plan of care.
Communicates with the physician regarding the patient's needs and reports any changes in the patient's condition.
Identifies discharge planning needs and implements prior to discharge Conducts admissions, recerts, ROC, and discharges for the assigned case load.
Conducts routine nursing visits as needed.
Conducts supervisory visits when necessary to monitor home health aide performance.
Responsible for the quality outcome scores of the assigned patients.
Responsible for the patient satisfaction scores of the assigned patients.
Responsible for the financial management of all assigned cases including insurance authorizations and meeting reimbursement requirements.
Demonstrates Standards of Performance (ownership, teamwork, communication, compassion) that support patient satisfaction and principles of service excellence.
All other duties as assigned.
Education: ADN from approved school required.
Bachelor's degree preferred.
Licensure: Registered nurse with current licensure to practice professional nursing in the state of North Carolina.
BLS required.
Valid Driver's License required.
Minimum 1 year of recent acute care, home health or related experience required.
Other - Hours of Work: Flexible hours to meet educational, departmental and institution needs.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
